Adult Foster Care
At Boston Home Care, our Adult Foster Care (AFC) program provides individuals with a safe, nurturing, and family-like environment where they can receive daily assistance and compassionate supervision. This service is designed for adults who, due to medical, physical, or cognitive limitations, require support with their activities of daily living but prefer to remain in a home rather than a facility.
Our AFC program pairs qualified caregivers with participants in approved home settings. Caregivers provide assistance with personal care, meal preparation, medication reminders, and companionship, ensuring each individual’s comfort, dignity, and overall well-being. Regular nursing oversight and ongoing support from our care coordination team help ensure that every participant’s care plan meets their evolving needs.
